Please fill out the form below to sign-up for the Wednesday night Family Supper.
Deadline: Noon on Tuesday
Family Limit: $15.00 (This applies to persons living in the SAME HOUSEHOLD ONLY).
You are obligated to pay for unused reservations if not canceled by noon on Tuesday.
Children (5th grade &under) at $1.50 per child.
Youth (6th-12th grade) at $3.00 per person.
Adults at $5.00 per person.
